TRAVEL FORM
As a participant in a group study experience, you are required to complete this form 10 days before you plan to travel outside of Rome. The purpose of this form is only to inform Gustolab International/Borromini Institute of your destination, itinerary and contact names/number, and to remind you of the importance of taking personal responsibility for your safety at all times.
You are asked to confirm that you have assessed risks & costs of running into restrictions regarding Covid-19 travel, testing and quarantine requirements, that you take full responsibility for your travel plans, that you have checked feasibility of your travel plans, including checking with your local contacts, that you will use the buddy system when going out, and inform your group and our staff of any change in your plans.
Please carry proper identification and the name, address and telephone number of any hotels/hostels/etc that you will stay at with you all times. This should be written in the language of the area where you are staying if you are not proficient in the language.
PAST FORM SUBMISSIONS
